The place is Earth, five thousand years after World War III. Heiro, a warrior/priest from the Abbys in what used to be Canada, travels South to the huge inland sea that used to be the Great Lakes. He's on a mission is to bring back a lost secret of the ancients, which may help his people defeat the Unclean. The unclean are those humans who have chosen to use the destructive knowledge and weapons of the past - and they've created hordes of mutated, intelligent beasts to do their dirty work.
Accompanying Hiero is his great riding moose (horses seem to be extinct in the future) and a young bear who is his friend. He communicates with them telepathically - and the power he possesses grows on his journey. He makes his way past all kinds of monsters and comes to the kingdom on the East Coast (Delaware area), where he finds his future wife.
This is a fun book, reminding me of everything from Planet of the Apes to Kamandi to those Gene Roddenberry movies (Genesis II, Planet Earth).
